A CASE OF GUILT

Female – 36 Year Old.

Name – Mrs K.G.

Occupation – Housewife.

Date of Case taking – 05 March 2021.

Doctor – Yes can u tell me what is happening ?

Pt – I have Depression and I am under treatment..

Urging to pass stools urgently as soon as I have the food…

Stitches in the rectum

Itching in the anal region…

Pain in the heels..

Had Fracture of the Right hand – < too much bike riding…

Headache all the time… < sun < hunger < food not on time.

<tension. < kid studies…

I take an allopathic painkiller… I feel better by it..

Doctor – Can u tell me when did your complaint start ?

Patient- My Mother In Law expired…so since that time I m having depression…

She expired one month after my baby took birth…

I took care of my mother in law…..she had cancer…

I was not well in that phase also…

I used to do all the work in home…

I had to manage two kids…

I had to Manage food…

Patient suddenly changed the topic and shared about her mindset currently –

Even if anyone tells me anything…opposite…etc…

I think a lot…

Everyone has their views…so I think a lot about it…

I get headache becoz of it…

I also get aggressive too soon…

Also doctor my Intercourse is usually unsuccessful….is it because space in my private parts increased in size after operation ? and is it because the stitches were not well taken ??

Dr – how it started…

Patient – I came to Kolhapur from ichalkaranji(a small village near main city)…

I had been to my village for delivery.

I was in Kolhapur for helping my mother in law and taking care of her.…

My first son was in nursery class …

My Mother in law’s behaviour… was very talkative…aggressive…village type criticizing all the time… but she was kind on me..

At times She used to taunt/mock a lot…

My Father In Law was good and calm in nature…

Now My Grandmother is not anymore. She is not alive..

Observation – Suddenly patient changed the topic and started sharing about her Depression

When I was in depression…

My mothers brother is a policeman… my Mother In Law was in good terms with him…

I used to feel that since my mother’s brother is a policeman, he has sent ppl to follow me…observe me

Because I did not serve my mother-in-law well…

No one tell me all this… but I think that is one of the main reasons, I have this depression..

DR – Tell me more …

Patient – I would like to tell you about My Father in law’s accident…

We were in Delhi..

My Mother in law and me crossed the road…she told me lets go…and my Father in law will come later…

And then suddenly my Father in law crossed the road and had an accident…

And in reality I was going to admit my Mother in law to hospital for some issue…

Patient pauses for 1 min.

Then told – I think i told you this story just like that..

Patient again pauses for 1-2 minute.

Patient I don’t blame anyone..

Patient looks around in the clinic . takes pause for 1-2 minute more..

Patient – Dr My weight is also increasing…

About my depression , some Dr told it is post partum depression… Do u Also think and diagnose my condition as post partum depression ?

But now it has been 12 years since my second delivery. I don’t think 12 yrs after delivery the depression lasts so long.

Dr – Can u tell me about yourself?

Patient – If anyone replies or I tell anything on the spot…

Dr -Can u Give me an example…patient couldn’t give example…

Patient pauses for a minute.

Dr – Anything more ?

Patient – I keep thinking how will things go about…

I m taking allopathic medicines from 2012….yet no change…

We did everything related to medicine and we even did some superstition related things … but not better as yet…

My husband also supports me well…so atleast that support I have.

Also If I go to someone’s place…no one can tell anything has happened to me , I feel and behave so normally..…but internally I feel there are lot of things happening which are not good , these thoughts and all…

Dr –can u tell me your behavior before all these incidents/ delivery that happened. Before the depression feeling/ thoughts started

Patient – I used to be housewife…my husband wanted this type of lady only…

My father was telling not to do this/that…don’t wear this / that…

There were lot of restrictions in my family.

Dr – can u tell me how many siblings do u have ?

Patient – We are 4 siblings – 2 brothers.. 2 sisters(including me)…

I am the eldest…

Dr – Can u tell me how is everyone’s behaviour ?

Patient – All are aggressive..

Then she changed topic and shared again about her father.

My father has non-vegeterian hotel…people used to tell me I get angry more because I eat nonvegetarian food… Is it true doctor ?

I also used to get moved by more emotional things here and there…

Patient takes pause for 1-2 minutes.

Patient – Doctor I also have this Leucorrhoea – if I don’t wash…I get itching more…immediately…

It is Whitish in nature.

Also I do not have Menses since – 2-3 years…

First was normal delivery…second was LSCS…

Dr – why did u need to do a caesarean section ?

I had more stitches after first delivery…so dr told to do LSCS…

Also after first delivery my Gastric tract has got disturbed…

Dr – can you tell me about the following each –

Likes – sweets ++ …vada pav(a Mumbai/maharashtra Snack having boiled mashed Potato with onion and some condiments fried and rolled in a bread)

That too I like specific good quality vada pav only and not any roadside one..

Dislikes – Bitter Foods I don’t like…nor too much spicy…

Stool – good , no issues

Urine – good but frequently I have to go…

Coughing << urine spurts..

Sleep – too much….good…

Dreams – relatives.. but not remembering….

Perspiration- Axilla…hollow of knee…

Below breasts…

Stains White

Dr I have become forgetful….

But by eating mirchi(green chilly) – I have got the habit of remembering well…

Dr -Can u tell me about your Anger?

Pt – My talk/point should be heard…

Also I have two types of thoughts at times –like I spent on straightening of my hair but I feel why I did it…I spent so much….why did I spend…etc…

But then I myself agree to the fact that I got it done and then there is no way I can undo it now.

Dr – can you tell me what weather do u not like ?

Patient – I cannot tolerate cold weather…

Dr – when there was periods/menses what were the symptoms before/during/after…

Patient – I did not get periods every month…since beginning of the menses in teenage.…

After marriage I used to get them regularly…

I used to have back and leg pain…

Bleeding more all through 4-5 days.…

Daytime bleeding more than night…

Sometimes I used to get leucorrhoea which is whitish , milk like in nature as well.

Dr – What do u feel on getting up in morning…

Patient – I get hunger as soon as I wake up…

Not fresh feeling…

Observations and analysis – The Repertory chosen is Reliable Repertory since it is a Recently released repertory with original sources/ reference string mentioned for authenticity/reliability of the rubrics as well as remedies added to each rubric –

1. The Patient was loquacious as well as her talks kept wandering the way they appeared in her mind.

Aur met 200 – 1 dose stat.

Placebo for 15 days.

Follow up visit analysis and prescription –

18^th March 2021 – Says she is 25% better.

Sleep is good. I had some negative Dreams but I m thinking positive My thoughts are much better, Digestion is better also. Overall I m feeling good.

Remedy – Placebo.

20^th March 2021 – cough has started since 2 days and in the night it was more.

I cannot breath when the cough is more…

Expectoration not coming out.

Day time today it started.

Yesterday it was not there… I took some turmeric & honey but not better…

Thirst- 4-5 times per day.

Acute remedy to be thought as a Relationship of aurum.

Remedy – Hepar Sulph 200 -1 powder stat. 2 Later sos if needed(cough , throat pain more) and

Placebo.

22 March 2021 – Patient got better in acute symptoms.

15^th April 2021 – Acidity too much.

Has to eat something.

Urination involuntary < more coughing = This symptom is under control now.

Urethral itching is much better now..

Doctor All things should not happen again which I had earlier.

Appetite intake for Food is good

Sleep is good nowadays.

Aggressive talks at times.

Mind wise I feel much better.

Remedy –

Aur Met 200 – 1 Powder stat.

And Placebo for 15 days.

06 May 2021 –

Patient feels much better so far.

Almost 50% better.

12 may 2021 –

Dr – Can u tell me in writing what changes have you felt after the homeopathic medicine.

Patient – My Acidity is better but evening it is more. Thoughts are coming regularly . I always feel there is someone behind me atleast once through the day. But I can now divert my mind.

Dr – Do u feel there is any difference as compared to earlier ?

Patient – Thoughts are in control now. Body is also recovering.

I cannot answer on the spot to anyone who tells me anything. Then I keep thinking about it. I should have told that person – this is what I feel. But later on if I meet that person I am not able to speak to him/her.

Dr- What is the Percentage of change as compared to earlier ?

Patient – 50% better as compared to earlier.

Dr – Okay. We will cover more in future treatment.

Patient – okay.
